Job summary

The Caring Connections in New York is seeking an experienced Patient Service Coordinator to join our home care team. You will manage a caseload, coordinate with nurses and caregivers, and ensure outstanding client service.

The role requires familiarity with HHA Exchange software, strong communication, and teamwork to support seniors and their families.

This is a full-time, on-site position with a friendly, supportive work environment.

Qualifications

• Experience as a Service Coordinator or similar in home care or nursing home settings.
• Familiarity with HHA Exchange software.
• Excellent communication, documentation, and follow-up skills.
• Strong teamwork and positive attitude.

Responsibilities

• Manage a reasonable-sized caseload and ensure services meet client needs.
• Communicate professionally with clients, families, and caregivers.
• Coordinate with nurses and caregivers for guidance and assignments.
• Use HHA Exchange to manage schedules, documentation, and communication.
• Participate in weekly staff meetings and share caseload updates.
• Answer calls professionally and maintain organized documentation.
