Responsibilities

• Assign and schedule caregivers for clients at home or in facilities.
• Confirm caregiver availability and secure acceptance of assignments.
• Conduct intake process and build patient profiles to facilitate caregiver matching.
• Ensure all cases for the following day are fully staffed before logging off.
• Maintain accurate scheduling records in HHA Exchange, Salesforce, and other systems.
• Inform caregivers of pertinent patient information and notify clients/families of assignments.
• Respond to voicemail and email messages within 24 hours or less.
• Document all communications accurately and consistently.
• Collaborate with the clinical team for any reported change in condition or clinical issues.
• Work with HR to inform caregiver demand and support recruitment efforts.
• Follow and enforce compliance with all regulations and agency policies.
• Communicate scheduling changes, client concerns, and missed visits to contract case managers promptly.
• Maintain up-to-date patient and caregiver schedules in required systems.
• Participate in on-call service duties including after-hours and weekend calls.
• Accept all new referrals unless a denial is authorized by leadership.

Position Overview: The Patient Services Coordinator is responsible for the coordination and scheduling of caregivers for clients receiving home care services. This role ensures that all cases are staffed appropriately, documentation is accurate, and communication with clients, caregivers, and departments is timely and professional. The coordinator also plays a key role in maintaining compliance with agency policies and contractual requirements.
