Job summary

PACS is seeking a Staff Occupational Therapist to evaluate and treat residents, develop individualized plans, and guide rehabilitation at a post-acute care facility. You will collaborate with physicians, nurses, and families to optimize outcomes and discharge planning.

You will supervise Certified Occupational Therapy Assistants, provide staff in-services, and ensure compliant documentation across the care team. The role emphasizes safety, education, and ongoing quality improvement initiatives.

Qualifications

• Education and/or Experience: Bachelor’s degree in Occupational Therapy; Masters/Doctorate preferred.
• Proficient computer skills in Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine occupational therapy needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance, endurance and independence.
• Supervise Certified Occupational Therapy Assistants and support staff.
• Communicate with physicians, nursing staff, residents and families regarding progress and discharge planning.
• Participate in Resident Care conferences and Rehabilitation meetings.
• Provide in-services and training to facility staff.
• Complete timely, accurate documentation in accordance with policies and payer requirements.
• Maintain a safe treatment environment and educate residents, families and caregivers.
• Recommend and facilitate ordering of adaptive equipment and assistive devices.
