Job summary

PACS is seeking a Staff Occupational Therapist to evaluate and treat residents, coordinate with families, physicians, and the health team, and maintain compliant documentation. You will develop and implement individualized treatment plans to improve mobility, balance, endurance, and independence, supervise COTA and aides, and provide in-service training.

Qualifications

• Bachelor’s degree in Occupational Therapy required; advanced degree preferred.
• Proficiency with Net Health and Point Click Care documentation.
• Ability to read and implement policy and procedure manuals.

Responsibilities

• Evaluate residents to determine OT needs and potential for rehabilitation.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and aides in resident care and activities.
• Communicate with physicians, nursing, families, and team members about progress and discharge planning.
• Complete timely, compliant documentation per facility and payer requirements.
• Provide in-service training and participate in care conferences and quality initiatives.
