Job summary

FOREST GROVE POST ACUTE is seeking an Occupational Therapist to evaluate residents, develop treatment plans, and improve mobility and independence. You'll supervise COTA and aides, train staff, and maintain documentation while collaborating with physicians, families, and the care team.

Requirements include state OT licensure, a bachelor’s degree in OT (Masters/Doctorate preferred), and CPR certification as applicable.

Qualifications

• Bachelor’s degree in Occupational Therapy required; Master’s/Doctorate preferred.
• Licensed OT in the state with ongoing CE requirements.
• CPR certification may be required by licensure.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and aides; provide training to staff on OT-related care.
• Maintain compliant documentation and communicate with physicians and families.
• Contribute to quality improvement initiatives and facility rehab program development.
