Job summary

Forest Grove Post Acute is seeking a Staff Occupational Therapist to evaluate and treat residents, communicate with families and physicians, and maintain documentation in the medical records.

You will develop, implement, and update individualized treatment plans to improve mobility and independence, supervise OT Assistants, and participate in care conferences and rehabilitation meetings.

Qualifications

• Bachelor’s degree in Occupational Therapy; masters or doctorate preferred.
• Proficient computer skills; inputting data into Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
• Supervise Occupational Therapy Assistants in direct resident care and related activities.
• Communicate regularly with physicians, nursing staff, residents, and families regarding progress, goals, and discharge planning.
• Participate in Resident Care conferences, Utilization Review meetings, and Rehabilitation meetings as needed.
• Provide in-services on training programs for other staff in the facility as needed.
• Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
• Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
• Assist in ordering necessary durable medical equipment to facilitate independence in ADLs.
