Job summary

Forest Grove Post Acute is seeking an Occupational Therapy Assistant (COTA) to support the staff OT in providing resident-centered care. You will help with treatment, documentation, and equipment needs while maintaining safety and infection control standards.

Ideal candidates hold an Associate Degree from an accredited COTA program, are licensed in the state, and can communicate effectively with residents, families, and the care team.

Qualifications

• Associate Degree from an accredited COTA program.
• Proficient with Net Health and Point Click Care systems.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to present information clearly to managers, coworkers and families.

Responsibilities

• Treat residents as directed by the Occupational Therapist.
• Complete timely, accurate, and compliant documentation.
• Maintain a safe treatment environment following infection control protocols.
• Participate in Resident Care conferences and rehabilitation meetings.
• Assist nursing with training of Restorative Aides.
• Communicate progress and discharge planning updates to the team.
• Provide in-services on training programs for staff as needed.
• Educate residents and caregivers on home programs and safety.
• Recommend durable medical equipment to support independence.
• Maintain confidentiality of residents and company information.
