Job summary

FOREST GROVE POST ACUTE in Oregon seeks an experienced Staff Occupational Therapist to evaluate and treat residents, develop treatment plans, and document care. You will train staff and collaborate with physicians, families, and the health team.

Must be licensed in Oregon, have a OT degree (Bachelors minimum; Masters/Doctorate preferred), and proficient with Net Health and PointClickCare. We offer comprehensive benefits and a supportive work environment.

Qualifications

• Licensed OT in Oregon or eligible for licensure in Oregon.
• Ability to read technical procedures and policy manuals.
• Ability to present information clearly to managers, coworkers and families.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise assistants and train staff as needed.
• Communicate with physicians, nursing staff, and families on progress and discharge goals.
• Complete timely and compliant documentation per policies and payer requirements.
