Job summary

FOREST GROVE POST ACUTE is seeking a Staff Physical Therapist to evaluate residents, develop and implement treatment plans, and supervise PTAs and aides. You will collaborate with physicians, nurses, and families to achieve rehabilitation goals and ensure compliant documentation.

The role requires a Bachelor’s degree in Physical Therapy, state licensure, CPR certification if required, and proficiency with Net Health and Point Click Care systems.

Qualifications

• Licensed as a Physical Therapist in the state.
• CPR certification, if required by licensure.
• Continuing education requirements per state practices.
• Proficient computer skills for Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
• Supervise Physical Therapy Assistants in direct resident care and resident related activities.
• Communicate progress with physicians, nursing staff, residents and families regarding goals and discharge planning.
• Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
• Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development.
