Job summary

Forest Grove Post Acute in Oregon is seeking a Staff Occupational Therapist Assistant to support the Occupational Therapist with resident care and treatment planning. The role emphasizes accurate documentation, adherence to regulatory requirements, and collaboration with the interdisciplinary team to enhance resident outcomes.

You will contribute to infection control, equipment maintenance, and ongoing education for residents and families on care plans and home exercise programs.

Qualifications

• Associate degree from an accredited COTA program.
• Must be licensed as an Occupational Therapist Assistant in Oregon.
• CPR certification may be required per licensure.

Responsibilities

• Assist the Staff Occupational Therapist with resident care and treatment planning.
• Complete timely, accurate documentation in accordance with facility, governmental, and third-party payer requirements.
• Maintain a safe treatment environment following infection control and equipment maintenance protocols.
• Participate in Resident Care conferences and Rehabilitation meetings as needed.
• Educate residents, families, and caregivers on treatment goals, home exercise programs, and safety techniques.
• Assist in training Restorative Aides and coordinate with nursing staff on discharge planning.
