FOREST GROVE POST ACUTE is seeking a Staff Occupational Therapist Assistant to support the Occupational Therapist with resident-focused care in a post-acute setting. The role involves direct resident care, documentation, and collaboration with the interdisciplinary team to plan goals and discharge progress.

The ideal candidate holds an Associate Degree from an accredited COTA program, is licensed as an OTA in the state, and can use Net Health and Point Click Care.

Qualifications

• Associate Degree from an accredited COTA program.
• Proficient in computer data entry for Net Health and PCC.
• Licensed as an Occupational Therapist Assistant in the state.
• CPR certification, if required by licensure.

Responsabilités

• Treat residents as directed by the Occupational Therapist.
• Complete timely, accurate, and compliant documentation.
• Maintain a safe treatment environment with infection control protocols.
• Participate in Resident Care conferences and Rehabilitation meetings as needed.
• Assist nursing with training Restorative Aides.
• Communicate with supervising OT regarding progress, goals, and discharge planning.
• Educate residents, families, and caregivers on treatment goals and home programs.
