Job summary

Optum is seeking an Advanced Practice Clinician (NP/PA) for Senior Community Care in the WA & Multnomah Counties, OR area. This field-based role offers a flexible schedule with no on-call, no weekends, and a focus on value-based care for medically complex patients in skilled nursing facilities.

You will manage your own patient panel, coordinate care across facilities, and lead advanced care planning discussions with an interdisciplinary team.

Qualifications

• Must hold an active, unrestricted NP license in the state of residence, or ability to obtain by start date.
• Board certification through AANP or ANCC for NP, or NCCPA for PA.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions with diagnostics and interventions.
• Develop and manage complex treatment plans.
• Lead care planning discussions with patients and caregivers.
• Collaborate with PCPs and interdisciplinary team members.
• Ensure accurate and complete clinical documentation.
