Optum is seeking an Advanced Practice Clinician (NP/PA) for Senior Community Care to provide longitudinal care to medically complex patients across Washington and Multnomah Counties. As the primary clinician for your panel, you’ll manage your own schedule in a field-based role with no call, no weekend or holiday requirements, and no RVU targets.

Qualifications

• NP: Graduate of accredited MSN or DNP program.
• NP: Active and unrestricted NP license in state of residence or ability to obtain by start date.
• NP: Board certified through AANP or ANCC in an approved NP specialty.
• PA: Graduate of accredited Physician Assistant program.
• PA: Active and unrestricted PA license in state of residence or ability to obtain by start date.
• PA: NCCPA board certified.

Responsabilités

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions with diagnostics and interventions.
• Develop and manage complex treatment plans.
• Care for patients during short-term sub-acute rehabilitation episodes.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with PCPs, facility staff, caregivers, and interdisciplinary team members.
• Provide patient and caregiver education.
• Ensure accurate and complete clinical documentation.
