Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) for Senior Community Care in Multnomah and Clackamas Counties, OR. This full-time, field-based role offers flexibility to tailor a schedule around patient needs, with a focus on quality outcomes and long-term patient relationships.

You’ll conduct comprehensive assessments, manage acute and chronic conditions, and develop complex treatment plans in community-based settings including senior living and private homes.

Qualifications

• Graduate of an accredited MSN or DNP program.
• Active and unrestricted Nurse Practitioner license, or ability to obtain by start date.
• Board certification through AANP or ANCC in an approved specialty.
• Graduate of an accredited Physician Assistant program.
• Active and unrestricted Physician Assistant license, or ability to obtain by start date.
• NCCPA certification.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions.
• Develop and oversee complex treatment plans.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with PCPs, caregivers, and interdisciplinary team members.
• Provide patient and caregiver education.
• Maintain accurate clinical documentation.
