Job summary

Optum in Washington & Multnomah Counties, OR, is seeking a Senior Advanced Practice Clinician (NP/PA) to provide longitudinal care in skilled nursing facilities. You will be the primary clinician for your patient panel, with autonomy to plan and execute care within a value‑based model.

You’ll collaborate with PCPs and an interdisciplinary team, manage complex cases, and maintain ownership of care plans while benefiting from strong support and no RVU quotas. This role is field-based and full-time.

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted NP license in the state of residence, or ability to obtain by start date.
• Board certification through AANP or ANCC in an approved NP specialty.
• Graduate of an accredited Physician Assistant program.
• Active and unrestricted PA license in the state of residence, or ability to obtain by start date.
• NCCPA certification.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
• Ability to obtain a collaborative practice agreement, if applicable.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions, diagnostics, and interventions.
• Develop and oversee complex treatment plans.
• Manage patients during short-term sub-acute rehabilitation stays.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with PCPs, caregivers, facility staff, and interdisciplinary partners.
• Provide patient and caregiver education.
• Ensure accurate and complete clinical documentation.
