Job summary

Optum’s Senior Community Care team is seeking an Advanced Practice Clinician (NP/PA) to serve as the primary clinician for patients across Washington & Multnomah Counties, OR. This field-based role offers autonomy within an interdisciplinary team, prioritizing quality outcomes in a value-based care model.

Responsibilities include comprehensive assessments, managing acute and chronic conditions, complex treatment planning, and goals-of-care discussions.

Qualifications

• MSN or DNP from an accredited program.
• Active NP license (or ability to obtain).
• AANP or ANCC certification in an approved NP specialty.
• Graduate of an accredited PA program.
• Active PA license (or ability to obtain).
• NCCPA certification.
• DEA licensure/prescriptive authority (or ability to obtain).

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions.
• Develop and oversee complex treatment plans.
• Provide care during sub-acute rehabilitation episodes.
• Lead goals-of-care and advance care planning discussions.
• Collaborate with PCPs, facility teams, and caregivers.
• Educate patients and families.
• Maintain accurate clinical documentation.
