Job summary

Optum is seeking an Advanced Practice Clinician (NP/PA) for Senior Community Care in Multnomah and Clackamas Counties, OR. This field-based role focuses on medically complex patients in long-term care settings and private homes.

The role emphasizes relationship-based care, independent patient panels, and collaboration with PCPs and an interdisciplinary team to improve outcomes and quality of life.

Qualifications

• Graduate of an accredited MSN or DNP program.
• Active and unrestricted Nurse Practitioner license or ability to obtain by start date.
• AANP or ANCC certification in an approved specialty.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions, including diagnostics and interventions.
• Develop and oversee complex, individualized treatment plans.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with interdisciplinary team members, caregivers, and community partners.
• Provide patient and caregiver education.
• Maintain accurate clinical documentation.
