Job summary

Pace Runners in Ohio is seeking a Physical Therapist to join an interdisciplinary PACE team based at the center with home visits. The role emphasizes clinical judgment to improve mobility and independence for older adults, rather than traditional productivity metrics.

You will work across settings, develop individualized plans, supervise PT assistants, and enjoy strong benefits including holidays, PTO, CEU support, life and disability insurance, retirement match, and mileage reimbursement.

Qualifications

• Must hold a current PT license within the state you are practicing in.
• Ability to work across care settings and travel locally as the role requires.
• Experience with older adults, home-based care, geriatrics, complex care, or interdisciplinary practice is valued.

Responsibilities

• Complete PT assessments and treatments using evidence-based techniques to maximize mobility and quality of life.
• Improve mobility, strength, balance, and independence through exercise, gait training, and modalities.
• Create individualized plans prioritizing safety, independence, and patient goals.
• Document outcomes and track progress to improve therapy reach.
• Recommend DME, orthotics, prosthetics, or other adaptive solutions as needed.
• Develop transitional restorative and wellness programs to sustain function.
• Build trusted relationships with participants, caregivers, families, and staff.
• Provide clinical guidance to Physical Therapist Assistants.
• Deliver therapy across the PACE Center, participants' homes, SNFs, and hospitals.
