Job summary

Pace Runners is seeking a Physical Therapist to join a PACE team focused on helping older adults stay independent at home. The role includes therapy at the PACE center and in participants’ homes, using clinical judgment to support mobility and safety.

A $5,000 sign-on bonus is offered, with opportunities for additional quarterly bonuses. You will work within an interdisciplinary team, provide patient-centered care, and guide treatment plans across settings to maximize function and quality of

Qualifications

Must hold a current PT license within the state you are practicing in.
Ability to work across care settings and travel locally as the role requires.
Experience with older adults, home-based care, geriatrics, complex care, or interdisciplinary practice is valued.

Responsibilities

Complete PT assessments and treatments to improve mobility and quality of life.
Improve mobility, strength, balance, and independence through therapeutic interventions.
Create individualized plans prioritizing safety and daily function.
Document outcomes and track progress to improve therapy delivery.
Recommend durable medical equipment and assistive devices as appropriate.
Develop transitional restorative and wellness programs for long-term function.
