Job summary

myPlace Health is seeking a Physical Therapist to join our PACE program and work as part of an interdisciplinary team to deliver personalized therapy, educate participants and families, and promote mobility and independence in home and center settings.

You will lead therapeutic exercises, assess mobility, and collaborate with nursing homes, skilled facilities, and assisted living teams to ensure comprehensive rehabilitation and safe practice.

Qualifications

• Graduation from an APTA-accredited Physical Therapy program.
• Active PT licensure in California in good standing.
• CPR and First Aid certification within 60 days of employment.
• Ability to lift up to 50 pounds.
• Driving to facilities may be required.

Responsibilities

• Collaborate with IDT meetings to assess and develop care plans.
• Assess mobility and functional abilities to create therapy plans.
• Deliver therapeutic exercises and gait training.
• Develop engaging individual and group exercise programs.
• Provide in-home therapy and collaborate with care facility staff.
• Document interventions accurately and review care plans.
