Pace Runners is seeking a Physical Therapist to join a PACE-based team focused on helping older adults stay independent at home and in the community. The role blends in-center care with home visits, applying clinical judgment to advance mobility and safety.

You will work with an interdisciplinary team in a dynamic, patient-centered environment, traveling locally to serve participants and coordinate care across settings, including homes and centers.

Qualifications

• Must hold a current PT license within the state you are practicing in.
• Ability to work across care settings and travel locally as the role requires.
• Experience with older adults, home-based care, geriatrics, complex care, or interdisciplinary practice is valued; exceptional clinicians from all settings are encouraged to apply.

Responsabilités

• Perform PT assessments and treatments to improve mobility and quality of life.
• Improve mobility, strength, balance, and independence through therapeutic exercise and gait training.
• Create individualized plans prioritizing safety and participant goals.
• Document outcomes and track progress.
• Recommend durable medical equipment or assistive devices.
• Develop transitional restorative and wellness programs for long-term function.
• Build relationships with participants, caregivers, and healthcare partners.
• Provide guidance to Physical Therapist Assistants.
• Deliver therapy across PACE Center, participants\' homes, and facilities.
