Job summary

Pace Runners in Cortland, OH is seeking a Physical Therapist to join an interdisciplinary PACE team based at the center with home visits, enabling participants to stay mobile in their homes. You will apply clinical judgment to support safety and independence for older adults.

This role offers a $5,000 sign-on bonus, collaboration with a broad care team, and comprehensive benefits including PTO, CEU allowances, retirement match, life insurance, and mileage reimbursement.

Qualifications

• Must hold a current PT license within the state you are practicing in.
• Ability to work across care settings and travel locally as required.
• Experience with older adults, home-based care, geriatrics, or interdisciplinary practice is valued.

Responsibilities

• Make a meaningful impact by completing PT assessments and treatment to improve mobility and quality of life.
• Improve mobility, strength, balance, and independence through exercise, gait training, and modalities.
• Develop individualized plans prioritizing safety, independence, and the participant's goals.
• Document outcomes and progress to optimize therapy delivery across settings.
• Provide clinical guidance to Physical Therapist Assistants and deliver therapy in center, homes, facilities, and hospitals.
