Job summary

Longterm Health Management Services in the Louisville Metro area seeks a licensed Physical Therapist to join a multidisciplinary PACE team, delivering PT services across home and center settings to improve mobility and safety for older adults.

You will assess function, plan therapy, educate patients and families, document progress, and collaborate with RNs, PCPs, OTs, social workers to optimize care.

Qualifications

• Licensed PT in Indiana or eligible for licensure.
• Minimum 1 year of geriatrics, home health, SNF, outpatient, or PACE experience.
• Excellent interpersonal and communication skills in an interdisciplinary model.
• Proficient in EMR systems and documentation practices.

Responsibilities

• Evaluate mobility, gait, balance, and rehab needs in home and center settings.
• Develop and modify individualized PT care plans with the team.
• Provide interventions to improve mobility, reduce fall risk, and independence.
• Educate participants and caregivers in exercises and assistive devices.
• Document assessments and progress per PACE standards.
• Participate in care planning meetings with RNs, PCPs, OTs, social workers.

Physical Therapist (PT) – PACE Program

About the Role:

We are seeking a passionate and experienced Physical Therapist (PT) to join a multidisciplinary care team at a reputable PACE (Program of All-Inclusive Care for the Elderly) facility in the Louisville Metro area. Key Responsibilities:

• Evaluate and assess participants’ physical function, mobility, gait, balance, and rehabilitation needs in both home and center environments.

• Develop, implement, and modify individualized physical therapy care plans in collaboration with the interdisciplinary team.

• Provide therapeutic interventions to help participants improve or maintain mobility, reduce fall risk, and enhance physical independence.

• Instruct participants, caregivers, and family members in therapeutic exercises, mobility techniques, and use of assistive devices.

• Document assessments, treatments, and progress in accordance with PACE program standards and regulatory guidelines.

• Participate in care planning meetings and coordinate closely with RNs, PCPs, OTs, social workers, and other team members.

Qualifications:

• Licensed Physical Therapist (PT) in the State of Indiana (or eligible for licensure).

• Minimum 1 year of experience in geriatrics, home health, SNF, outpatient, or PACE preferred.

• Excellent interpersonal and communication skills; able to thrive in an interdisciplinary care model.

• Proficient in EMR systems and documentation best practices.

• Valid driver’s license and reliable transportation (home visits may be required).
