Pace Runners is seeking a Physical Therapist to join our interdisciplinary PACE team. This role combines center-based care with home visits to assess and treat older adults in their homes and community settings.

You will make robust clinical decisions, focusing on safety, function, and participant goals, while collaborating with an interdisciplinary team to improve mobility and prevent decline.

Qualifications

• Must hold a current PT license within the state you are practicing in.
• Ability to work across care settings and travel locally as needed.
• Experience with older adults, home-based care, geriatrics, complex care, or interdisciplinary practice is valued.

Responsabilités

• Complete PT assessments and treatment to improve mobility and quality of life.
• Develop individualized plans prioritizing safety and independence.
• Document outcomes and track progress efficiently.
• Recommend DME, orthotics, and adaptive equipment as needed.
• Provide supervision to Physical Therapist Assistants.
• Deliver therapy across the PACE Center and participants' homes.
