Job summary

Placements24 is seeking a highly experienced Senior Support Coordinator to oversee and deliver mental health services in Queenstown, Eastern Cape. This leadership role entails managing a complex caseload, developing personalized plans, and coordinating care with a multidisciplinary team.

You will mentor junior staff, ensure high standards of care, and advocate for clients within health and social services. Travel locally may be required as part of the role.

Qualifications

• Master's degree in Psychology, Social Work, Counselling, or related mental health field.
• Minimum of 5 years of experience in mental health support, case management, or counseling.
• Proven experience in a senior or leadership role within mental health services.
• Registered with the relevant professional body (e.g., HPCSA, SACSSP).
• Valid driver's license and willingness to travel locally.

Responsibilities

• Manage a caseload of individuals with diverse and complex mental health conditions.
• Develop, implement, and review individualized mental health support plans.
• Coordinate care services, ensuring seamless transitions and access to resources.
• Provide counseling and therapeutic interventions as appropriate.
• Liaise with psychiatrists, psychologists, general practitioners, and other health professionals.
• Offer support and guidance to clients' families and caregivers.
• Mentor and supervise junior support workers and case managers.
• Ensure accurate and timely documentation of client progress and services provided.
• Advocate for clients' needs and rights within the healthcare and social services system.
• Participate in team meetings, case conferences, and professional development activities.
