Job summary

Placements24 is seeking a Remote Social Support Navigator to join our client’s innovative service delivery team. This fully remote role supports individuals and families in the East London and surrounding Eastern Cape areas, guiding clients through complex systems to access housing, healthcare, employment, and financial assistance.

You will conduct needs assessments, provide information, and connect clients with essential social services while maintaining confidential records and collaborating

Qualifications

• A relevant qualification in Social Work, Community Development, Human Services, or a related field.
• 2–3 years of experience in case management, social support, or community outreach.
• Strong understanding of social welfare systems in the Eastern Cape and East London.
• Excellent interpersonal, communication, and active listening skills.
• Proficiency with virtual tools and digital record-keeping.

Responsibilities

• Conduct comprehensive assessments of clients' social support needs, including housing, health, employment, and financial stability.
• Provide personalized guidance to help clients navigate and access community resources and social services.
• Educate clients about available programs, eligibility criteria, and application processes.
• Assist clients in completing necessary forms and applications for services.
• Maintain regular contact with clients via phone, email, and video conferencing to monitor progress and provide ongoing support.
• Collaborate with service providers and community organizations to advocate for client needs and coordinate services.
• Document all client interactions, assessments, and progress notes in the client management system.
• Identify gaps in service provision and provide feedback for program improvement.
• Maintain up-to-date knowledge of local social services, resources, and referral pathways.
