Telecare Corporation in San Diego recruits a Case Manager III to support clients with mental illness in achieving independence within their communities. You’ll provide direct case management under supervision and help coordinate services across teams.

The role emphasizes driving clients to appointments using the company vehicle and meeting defined position standards with additional education/experience as differentiators.

Qualifications

• Bachelor's degree in Social Services or Human Services and 1 year of experience OR High School Diploma/GED and 4 years of direct mental health/community services.
• Valid and current driver's license, with personal vehicle insurance listed for you.
• Willingness to use the company vehicle to transport clients to appointments and groups.

Responsabilités

• Serve as a primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Work under direct supervision to meet position standards and perform job duties.
