Job summary

Telecare Corporation is hiring a Case Manager III to support individuals with mental illness in San Diego, providing primary case management and assistance to live independently within the community. Under supervision, you will handle transportation, engage in housing support, and coordinate services with community partners.

The role requires a Bachelor's in Social/Human Services or equivalent experience, a valid driver's license, and willingness to use your own vehicle for client transport.

Qualifications

• Bachelor's degree in Social Services or Human Services with 1 year of experience or High School Graduation/GED with 4 years direct mental health or community services experience.
• Valid and current driver's license with personal vehicle insurance in your name.
• Willingness to use personal vehicle to drive clients to appointments and attend meetings.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness and co-occurring disorders to help them live as independently as possible in their community.
• Operate under direct supervision and progress to Case Manager III duties with additional education and experience.
• Coordinate with clients, families, and community resources to support housing stability and community integration.
• Perform required task functions and meet position standards as a Case Manager III.
