Qualifications

• High School Graduation or G.E.D. equivalent with 2 years' direct mental health/community services; or an Associate's with 1 year; or a Bachelor's in Social Services.

Responsibilities

• Provide case management and primary support to individuals with mental illness to live independently in the community.
• Coordinate services and supports across community resources.
• Assist with transportation to appointments and groups using personal or company vehicle.
