Qualifications

• Bachelor’s degree or 3 years related experience in human services, psychology, or public health.
• Excellent writing, speaking, and organizational abilities.
• Ability to manage multiple tasks in a fast-paced environment.
• Independent and team-working capabilities.
• Willingness to travel monthly out of Fairfield County.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide comprehensive medical case management including treatment adherence counseling.
• Follow Ryan White Part C program policies.
• Assist clients in obtaining medical and social services and coordinating care.
• Develop initial and follow-up assessments of service needs.
• Collect and report data for The HIV Program.
• Assist data collection from EMR into CAREWARE for grant requirements.
• Attend department and program meetings and staff calls.
• Collaborate with the Ryan White Part C Program Team.
• Perform other duties as assigned.
