Job summary

Positive Impact Health Centers in Georgia seeks a Medical Case Manager to provide medical case management to adult clinic patients under HRSA/HRSA Ryan White guidelines, working with an interdisciplinary team of medical providers, behavioral health staff, and support services.

The role includes biopsychosocial assessments, ISP development for about 70 patients, coordinating referrals and ensuring HIPAA compliance, with ongoing adherence and retention support for patients’ health outcomes.

Qualifications

• Strong social work, nursing, or case management skills.
• Excellent interpersonal and multi-cultural communication abilities.
• Proficient computer skills for word processing, data entry and EMR navigation.
• Knowledge of HIV medical and case management fundamentals preferred.
• Ability to work independently and in a team.

Responsibilities

• Learn and stay current on HIV/AIDS and case management issues.
• Provide medical case management to identified patients following HRSA/Ryan White guidelines.
• Complete biopsychosocial assessments using required tools.
• Develop ISP for an average caseload of 70 patients and monitor progress.
• Coordinate referrals and follow-up for medical and non-medical services.
• Document patient information accurately in the EMR system.
