Job summary

Positive Impact Health Centers (PIHC) is seeking a Medical Case Manager to provide comprehensive case management for adult clinic patients in a multidisciplinary setting. The role emphasizes adherence to HRSA/Ryan White guidelines and coordination with a team including medical providers and behavioral health professionals.

The MCM will complete biopsychosocial assessments, develop Individualized Service Plans, monitor progress, and coordinate services to meet patient goals, including medication

Qualifications

• Social service or nursing degree (BSW/MSW preferred).
• One year of experience in a service delivery setting is required.
• Professional experience working with adults living with HIV is preferred.
• Spanish language fluency is highly preferred.

Responsibilities

• Learn and stay current on HIV/AIDS and case management.
• Provide case management to identified clinic patients with ISP development.
• Coordinate referrals for medical and non-medical services.
• Maintain HIPAA and EMR documentation in a timely, accurate manner.
• Assist patients with applications for ADAP and pharmaceutical assistance.
