Job summary

You will lead weekly PDPM meetings, monitor PDPM tools, supervise MDS processes, train new coordinators, and coordinate interdisciplinary care to ensure timely, compliant documentation and optimal resident outcomes in a long-term care setting.

Qualifications

• Graduation from an accredited nursing program.
• RN license in good standing (though not required).
• Minimum 3 years supervisory experience; long-term care preferred.

Responsibilities

• Oversee weekly PDPM meetings for skilled patients.
• Monitor use of PDPM tools and data reporting.
• Provide PDPM and MDS training to new coordinators.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual).
• Coordinate interdisciplinary participation in completing the MDS for new admissions.
