Job summary

PACS Group is seeking an experienced MDS Coordinator to oversee the facility’s resident assessment process, including MDS, PPS Medicare, and care planning. The role emphasizes accuracy, regulatory compliance, and timely documentation.

You will lead weekly PDPM meetings, supervise staff, provide PDPM training, and analyze data to improve outcomes across the facility. This is a full-time on-site role located in the United States.

Qualifications

• Graduation from accredited nursing school; RN license preferred but not required.
• In good standing with the State Board of Nursing and ongoing education/licensing.
• 3 years of supervisory experience; long term care experience preferred.

Responsibilities

• Oversee facility MDS workflow, ensuring timely and accurate submissions.
• Lead weekly PDPM meetings and monitor PDPM tool utilization.
• Provide PDPM and MDS training to new coordinators and staff.
• Enter and verify data for MDS assessments and PPS Medicare reviews.
• Coordinate interdisciplinary care plan development and changes.
• Ensure confidentiality and regulatory compliance in documentation.
