Job summary

PACS in Poway, CA is seeking an experienced MDS/Nursing Supervisor to oversee the resident assessment process, including MDS, PPS, and care plans. You will lead weekly PDPM meetings, monitor documentation accuracy, and coordinate interdisciplinary input to ensure compliance and quality of care.

The role requires strong nursing leadership in long-term care, data-driven decision making, and training new MDS coordinators. RN licensure is encouraged and professional development is supported.

Qualifications

• Graduation from an accredited nursing program.
• RN license in good standing with the State Board of Nursing (though not required).
• 3 years of supervisory experience preferred.
• Long Term Care experience.

Responsibilities

• Oversees facility weekly PDPM meetings for skilled patients.
• Tracks and monitors PDPM tools and related processes.
• Monitors QM and QASP performance and provides data analysis and recommendations.
• Provides PDPM and MDS training to new MDS Coordinators.
• Assesses health status and level of care for new admissions.
• Ensures timely completion of all MDS assessments (PPS Medicare, quarterly, annual, etc).
• Coordinates interdisciplinary participation in MDS for new admissions.
