Job summary

White Blossom Care Center in San Jose, CA is seeking an experienced LVN Supervisor to join our leadership team. This role provides clinical oversight, mentors LVNs and CNAs, and ensures high-quality resident care in a fast-paced SNF.

The ideal candidate holds an active California LVN license, SNF experience, and proven supervisory ability. PCC audits, Title 22 compliance, and strong communication are essential. This is a full-time, on-site role with competitive pay and benefits.

Qualifications

• Current California LVN license in good standing.
• Direct SNF experience is required.
• 1–2 years of supervisory or charge nurse experience preferred.
• Proficiency with PCC audits.
• Strong knowledge of Title 22 and long-term care regulations.
• Excellent communication, leadership, and organizational abilities in a fast-paced setting.

Responsibilities

• Leadership: Supervise and support nursing staff (LVNs and CNAs), providing guidance and performance feedback.
• Clinical Oversight: Coordinate and complete resident admissions and all associated clinical documentation.
• Quality Assurance: Conduct and oversee PCC audits to ensure accuracy, clinical integrity, and compliance.
• Compliance: Ensure shift-wide adherence to state, federal, and facility regulations.
• Collaboration: Monitor resident conditions and work closely with the interdisciplinary team to update care plans.
• Operations: Assist with staffing, scheduling, and maintaining a safe, efficient, and compassionate environment.
