Job summary

White Blossom Care Center in San Jose, CA is seeking an experienced LVN Supervisor to lead our nursing team on the day shift. You will oversee LVNs and CNAs, ensure regulatory compliance, and drive high-quality resident care in a skilled nursing environment.

In this role, you will coordinate admissions, perform clinical audits in PCC, and mentor frontline staff while maintaining a safe, compassionate, and efficient facility operations.

Qualifications

• California LVN license active and in good standing.
• Direct SNF experience required.
• 1–2 years supervisory or charge nurse experience preferred.
• Proficient with PCC audits.
• Knowledge of Title 22 and long-term care regulations.

Responsibilities

• Supervise and support nursing staff (LVNs and CNAs).
• Coordinate admissions and complete clinical documentation.
• Conduct PCC audits for accuracy and compliance.
• Ensure shift-wide regulatory compliance.
• Collaborate with interdisciplinary team to update care plans.
• Assist with staffing, scheduling, and maintaining a safe environment.
