Job summary

Qualifications

• Graduate from an accredited school of nursing or bachelor’s degree with 4–6 years of related experience.
• Current RN or NP licensure per state requirements.
• RUG certification must be obtained within 90 days of hire.

Responsibilities

• Evaluate needs of high-risk members and formulate care plans.
• Develop and update long-term care plans with care teams.
• Coordinate between members, families, and providers for appropriate services.
• Monitor progress and revise plans to meet goals.
• Educate members and families on disease management and benefits.
• Act as liaison among member, physician, and facilities.
