Formación

• Graduate from an accredited school of nursing or a bachelor's degree with 4–6 years of related experience.
• RN state licensure and/or NP licensure required.

Responsabilidades

• Assess and coordinate all aspects of medical and supporting services for high acuity members.
• Develop personalized care/service plans for long-term care members.
• Educate members and families about services and benefit options.
• Collaborate with care management team to identify providers and community resources.
• Monitor and update care plans and member status; revise as needed.
• Ensure compliance with state, federal, and clinical guidelines.
• Act as liaison between members, families, physicians, and facilities.
• May perform home visits and participate in onboarding and coaching of new hires.
