The hybrid role requires in-home assessments in the Donna, Mercedes, or Harlingen area and collaboration with a care management team to ensure high-quality, cost-effective care.

Qualifikationen

• Registered Nurse licensure in Texas or compact state.
• A Nurse Practitioner license is accepted where applicable.
• RUG certification must be obtained within 90 days of hire.

• Assess care needs for high risk/high acuity members.
• Develop and monitor long-term care plans and coordinate with providers.
• Coordinate between members, families and care team for person-centered care.
• Monitor progress and update care plans to meet member goals.
• Educate members and families on services and benefits.
