Job summary

Centene is hiring a care management professional to assess and coordinate care for complex/high-acuity members in a hybrid role serving Harlingen, Mercedes, Weslaco areas. You will develop personalized care/service plans, educate members and families, and collaborate with providers to ensure high-quality, cost-effective care.

The ideal candidate holds a nursing degree (BSN) with 4–6 years of related experience and active RN/NP licensure, plus RUG certification within 90 days of hire.

Qualifications

• Graduate from an accredited nursing program with 4–6 years of related experience.
• RN or NP licensure required or active licensure in state; RUG certification within 90 days of hire.

Responsibilities

• Assess and coordinate care across the continuum for complex/high-acuity members.
• Develop and monitor long-term care plans with care management team.
• Educate members and families on services and benefits.
• Act as liaison between members, families, providers, and facilities.
• Ensure compliance with state/federal regulations and clinical guidelines.
