Job summary

Centene is seeking a RN care coordinator to assess and coordinate services across the continuum for complex adult members in Texas. The role includes home visits and remote work in a hybrid arrangement, with a standard Monday–Friday schedule.

Requires 4–6 years of nursing/case management experience and RN licensure; NP licensure may apply in some cases. Benefits include comprehensive medical, retirement plans, tuition reimbursement, and paid time off.

Qualifications

• Graduate from an Accredited School of Nursing.
• Requires 4–6 years related clinical nursing and/or case management experience.
• Bachelor of Nursing degree preferred; RN or NP licensure as applicable.

Responsibilities

• Evaluates the needs of complex/high risk members and recommends a plan.
• Develops long-term care/service plans and coordinates providers and resources.
• Coordinates between member, family/caregivers, and care team for person-centered care.
• Monitors and revises care plans to meet goals; tracks status and changes.
• Travel is required for member home visits.
