Job summary

Sullivan Park Care Center seeks an experienced LPN/LVN supervisor to provide direct nursing care and oversee nursing assistants. You will ensure compliance with policies, participate in the QA program, and assist in discharge planning and resident care coordination.

The role requires a nursing degree or LPN/LVN credential, current license, and CPR certification. Knowledge of PCC is preferred; this on-site position offers competitive hourly pay and benefits.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Current, unencumbered license to practice as an LPN/LVN in this state.
• Current CPR certification.
• Knowledge and experience with PCC preferred.

Responsibilities

• Provide direct nursing care to residents and supervise nursing assistants.
• Direct the day-to-day nursing functions in accordance with regulations and facility policies.
• Ensure nursing staff compliance with procedures and participate in QA and discharge planning.
