Job summary

CENTERVILLE in is seeking an LPN/LVN supervisor to provide direct nursing care and oversee nursing assistants in our long‑term care facility. You will ensure adherence to policies, supervise shifts, and participate in QA activities to maintain high standards of resident care.

The role requires an active nursing license, English proficiency, and leadership ability. CPR certification is preferred; PCC experience is a plus. This is a full-time, on-site position with healthcare benefits.

Qualifications

• Must possess a Nursing Degree or LPN/LVN diploma from an accredited program.
• Current, unencumbered license to practice as an LPN/LVN in this state.
• English language proficiency: read, write, speak, understand.
• Current CPR certification; PCC knowledge preferred.
• Leadership and supervisory ability required.

Responsibilities

• Direct day-to-day nursing assistants in line with regulations.
• Lead shift planning, services, and activities with staff.
• Ensure policies and procedures are followed by nursing personnel.
• Participate in quality assurance and facility surveys as needed.
• Coordinate discharge planning and maintain accurate documentation.
