Job summary

Twin Cities Physicians is seeking a Nurse Practitioner to provide advanced wound care within community-based settings across Minnesota, focusing on medically complex patients in senior living and post-acute care. The role blends wound care, chronic disease management, and senior-focused medicine in a team-based environment.

This full-time position offers a competitive total compensation package, mileage reimbursement, CME allowance, malpractice coverage, and DEA reimbursement, with base salary

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license required.
• Experience in wound care, geriatrics, primary/post-acute care preferred.
• Wound care certification highly valued.

Responsibilities

• Provide advanced wound care assessments and treatment planning.
• Conduct rounding visits across senior living and long-term care communities.
• Manage pressure injuries, diabetic wounds, and skin integrity concerns.
• Coordinate care with physicians, nursing staff, therapists, and leadership.
• Support chronic disease management and preventive care initiatives.
• Aim to improve outcomes and reduce hospitalizations.
