Job summary

Twin Cities Physicians in Minnesota is seeking an Advanced Wound Care Nurse Practitioner to serve senior living communities throughout the Twin Cities Metro. This role combines advanced wound care, community rounding, chronic disease management, and senior-focused medicine into a unique practice environment.

You'll work within assisted living, memory care, skilled nursing and long-term care settings, helping patients receive specialized care where they live rather than navigating multiple

Qualifications

• Active Minnesota CNP license; AGNP preferred.
• Experience in wound care, geriatrics, primary/post-acute care.
• Excellent clinical judgment; able to manage complex patients.
• Valid driver’s license and ability to travel across Twin Cities Metro.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits in assisted living, memory care, skilled nursing, and long-term care.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help improve outcomes while reducing avoidable complications and hospitalizations.
