Job summary

Twin Cities Physicians is seeking a Mobile Wound Care Nurse Practitioner to provide on-site rounding in senior living communities across Minnesota. AGNP/CNP certified NPs will manage complex wounds, develop treatment plans, and serve as a key clinical partner to staff, families, and physicians within assigned communities.

Qualifications

• Active Minnesota NP license (CNP/AGNP) required.
• Experience in geriatrics, primary care, or wound care preferred.
• Valid driver's license and reliable transportation.
• Passion for serving the senior population.

Responsibilities

• Provide on-site rounding and advanced wound care.
• Develop and manage individualized treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Communicate with families and community staff.
• Manage medications, labs, and documentation.
