Job summary

Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner to provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds across assisted living, memory care, skilled nursing, and long-term care communities in Minnesota.

You will travel within the Twin Cities Metro area, collaborating with physicians, nurses, therapists, caregivers, and facility leadership to improve healing and outcomes.

Qualifications

• Active Minnesota CNP license required (AGNP preferred).
• Experience in wound care, geriatrics, primary or post-acute care.
• Strong clinical judgment for medically complex patients.
• Valid driver's license and willingness to travel in Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments and ongoing management of complex wounds.
• Conduct rounding visits across assisted living, memory care, skilled nursing and long-term care communities.
• Manage medications, diagnostics, and treatment plans with follow-ups.
• Collaborate with physicians, nurses, therapists, caregivers and leadership.
• Identify barriers to healing and implement evidence-based interventions.
• Support continuity of care and improve clinical outcomes.
