Job summary

Twin Cities Physicians is seeking a full-time Wound Care Nurse Practitioner to deliver proactive, geriatric-focused care directly in assisted living, memory care, and long-term care communities across Minnesota. The role emphasizes early identification and intervention to prevent complications and hospitalizations, with on-site visits and strong collaboration with physicians and staff.

Compensation ranges from $120,000 to $160,000+ total (base salary plus productivity incentives), plus mileage

Qualifications

• Active Minnesota CNP license.
• Experience in wound care and geriatrics or related settings preferred.
• Strong clinical judgment and ability to manage complex patients.
• Valid driver’s license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management within senior living communities.
• Conduct routine rounding visits and follow-up care across assigned communities.
• Manage medications, diagnostics, treatment plans, and clinical interventions.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Identify risks early and implement strategies that help prevent complications and hospitalizations.
• Support continuity of care while improving healing outcomes and quality of life.
