Job summary

Presbyterian Villages of Michigan is seeking a Full Time Service Coordinator at The Village of Warren Glenn in Warren, MI. The role focuses on coordinating programs and services to help residents maintain a good quality of life and age in place.

The Service Coordinator will assess needs, connect residents with resources, and manage care plans while collaborating with community organizations to offer affordable and new services. This position supports independent living and resident wellbeing.

Qualifications

• Two+ years of experience in social service delivery with elderly and/or family population preferred.
• Demonstrated knowledge of supportive services and local resources.
• Ability to advocate, organize, problem-solve, and deliver results for residents.

Responsibilities

• Coordinate programs and services to help residents maintain quality of life and age in place.
• Assess resident and family needs and connect to outside resources.
• Coordinate home care services to support independent living and aging in place.
• Engage residents to identify needs and promote involvement in programs.
• Submit reports to HUD to maintain funding for the program.
